You may have questions like, “Will they accept my dental insurance?” or “Do you offer payment plans?” We DO accept most insurance plans, but you’re welcome to call our office at (219) 947-2581 and our staff will be able to verify that for you.

If you’re wondering about financing options before your visit, the answer to that is yes, we offer payment plans through our partner, CareCredit®. Be sure you ask our staff for more information at your visit!

What we need from you before your visit

• To give you the best service possible, we ask that you fill out a few forms before your appointment. This saves us both time and helps ensure that you’re seen on your actual appointment time (we don’t like to keep you waiting).
